Teacher Self-Report fill-in program

Field Trip Roster
Instructions
1.
Fill in the information on the form.  Please enter students, last name first, and don't forget the student ID.
2. Save the Word document to your computer with an appropriate filename (such as "Volleyball Team")
3. Attach the document to an e-mail and send it to Mr. McConnell.
4. On the day of the trip you must give the attendance clerk a list of the students who actually go on the trip.

Announcement
Instructions
1. Fill in the information on the form.
2. If you want a copy for yourself, click the print button at the top of the form.
3. Click the "Submit by Email" button at the top of the form.  Copies will be sent to Mr. Matthew and Mr. McConnell.  After administrative approval the announcement will be made on the dates you specify.

Grade Change
Instructions
1.
On the form enter all necessary information about the grade(s) you wish to change.
2.
Save the Word document to your computer with an appropriate filename (such as "Grade Change 3-3-07.")
3. Attach the document to an e-mail and send it to Kristin Hector.

RSCCC Requisition Form (Purchase Order)
Instructions
1.
On the form enter all necessary information.
2. Print two copies.  Keep one for your records and take the other to Kristin Hector.
